Popliteal artery - a fairly large vessel, directly continuing down the femoral artery. It lies in the composition of the neurovascular bundle, with the same name vein and tibial nerve. The back side of the popliteal fossa, the vein lies closer to the surface than the artery; tibial nerve – even more superficial than the blood vessels.

Location and topography

Starting at the lower aperture leading to the channel located under polymembrane mice, the popliteal artery is adjacent to the bottom of the popliteal fossa of the femur to the first (directly to the popliteal surface), and further – the shell of the capsule of the knee joint.

The Lower division of the artery in contact with the popliteal muscle. It penetrates into the narrow space between the bellies of the gastrocnemius muscle which cover it. And reaching the edge of the soleus muscle, the vessel is divided into posterior and anterior tibial arteries.

Popliteal artery
The direction of the popliteal artery in its course changes:
• In the upper part of the popliteal fossa, the vessel has a direction downwards and outwards.
• Starting from the level of the middle of the popliteal fossa, the popliteal artery is directed almost vertically down.

Branches of the popliteal artery

The path of the popliteal artery gives several branches:
• Upper muscular branches.
• Upper lateral knee artery.
• Upper medial knee artery.
• the Middle artery of the knee.
• lower side of the knee artery.
• lower medial knee artery.
• Calf arteries (two; rarely – more).

ligation of the popliteal artery

Aneurysm popliteal artery

According to medical statistics, it is the most common localization of aneurysms in the periphery: about 70% of peripheral aneurysms localized in the popliteal region. The main reason of this pathological state is considered to be atherosclerosis, because it is set as the etiological factor of the vast majority of patients with an aneurysm of the popliteal artery.
Aneurysm of the popliteal artery develops almost regardless of age; the average age of patients is approximately 60 years, and the range of ages – from 40 to 90 years. Bilateral involvement is recorded in 50% of cases.
Much more often the disease affects men.
The clinical picture is dominated by symptoms of ischemic lesions of the distal extremities; there may be symptoms of compression of the nerve and the vein (in the compression of their aneurysm).
Complications:
• thrombosis of the aneurysm (aneurysmal cavity);
• aneurysm rupture;
• calcining the aneurysm;
• compression of the nerve.
For diagnostics used:
• angiography;
• computed tomography.
For the treatment of the most frequently performed ligation of the popliteal artery on both sides of the aneurysm (proximal and distal to it), followed by bypass surgery.

Thrombosis popliteal artery

aneurysm of the popliteal arteryPredisposing factors for blood clots in the arteries is the lesion of the internal surface of the vessel, the reasons for which there may be the following factors:
• atherosclerotic deposits on the vessel walls.
• hypertension;
• diabetes mellitus;
• trauma to the vascular wall.
• vasculitis.

Clinical manifestations

Thrombosis of the popliteal artery manifested by the following features:
• soreness in the limbs that appear sharply. Patients often compare her appearance with a kick. In the future, the pain can take paroxysmal in nature; moreover, the onset of pain leads to the appearance of sweat on the skin. Some easing of the pain over time do not means objective improvement of the condition of the patient.
• Blanching of the skin of the affected limb.
• Lowered skin temperature of the affected limb.
• the Appearance of leg bulges; its location coincides with the level of localization of the clot.
• Lowered, and later – the disappearance of sensitivity in the foot; the appearance of paresthesias.
• limited mobility of the affected limb. In the future, the mobility may be lost completely.
As a rule, symptoms develop gradually, starting with the appearance of soreness.
In the absence of adequate measures can develop a complication of gangrene. This state is characterized by the presence of clear boundaries between normal and nekrotizirovannah fabrics. In the future, necrotizing plot modificeres.
Worst-case scenario – infection necrotizing of the site. This condition is diagnosed by dramatically developing hyperthermia, a marked leukocytosis in the blood and the presence of ulcerative decay.
