Pulmonary hypertension – is the generic name of a group of diseases characterized by progressive increase in pulmonary vascular resistance leading to right ventricular failure and, as a consequence, premature death. For the first time this disease was identified in 1891 by Dr. Ernst von Romberga, and in 1973 at the meeting who was the first to attempt classification of pulmonary hypertension. First divided pulmonary hypertension into primary and secondary. Primary hypertension, in turn, were divided into occlusive, arterial, net and thromboembolic forms. Later, in 1998, the second conference, held in évian-Les-Bains, it was proposed to classify pulmonary hypertension based on clinical manifestations. According to the latest classification there are five types of this disease:  venous, arterial, thromboembolic, hypoxic and combined.

The Development of pulmonary hypertension due to a gradual narrowing of the lumen of medium and small pulmonary vessels. The result of this narrowing is complete closure of the blood vessels, increasing  the pressure in the blood vessels of the lungs, deteriorating pumping function of the heart chambers. Pulmonary hypertension refers to a group of multidisciplinary complex diseases, and knowledge about them is constantly replenished.

Pulmonary hypertension: symptoms.

Patients complain of General weakness, fatigue, shortness of breath, squeezing pain and discomfort in the left side of the chest, fainting, and swelling of the feet. Symptoms may have a different intensity depending on the severity of the disease. There are four functional classes of pulmonary hypertension. The group with the first class of pulmonary hypertension includes patients who struggle with a strong physical exertion; to the second class of hypertension are patients who feel fine at rest, but even at moderate loads they have shortness of breath, weakness, pain in the chest and dizziness. Patients with third class of pulmonary hypertension experience the above symptoms even when the household loads. Fourth grade of pulmonary hypertension was classified in patients who are unable to tolerate the slightest physical exertion and even at rest they are suffering from weakness and shortness of breath.

An Important place among the causes of pulmonary hypertension is the most common type of lung disease – chronic bronchitis which occurs in 90% of smokers. In addition, pulmonary hypertension – the most common complication in damage to the heart muscle in patients with heart disease suffering from coronary artery disease and inflammatory diseases of the myocardium. Also, quite often this disease occurs in patients with various autoimmune pathologies such as rheumatoid arthritis, lupus and scleroderma.

Pulmonary hypertension: treatment and prevention.

Treatment of such serious diseases as pulmonary hypertension is one of the most difficult tasks physicians and cardiologists. Currently around the world created a number of specialized centres for the treatment and research of this disease. The treatment of this disease involves interventions aimed at reducing the likelihood of deterioration of disease. These include: contraception, vaccination against pneumococcal infection and flu, controlled rehabilitation, psychosocial support and dosing of physical activity.

The Main goal in the treatment of pulmonary hypertension is to eliminate the reasons for its occurrence, and also decrease blood pressure in the pulmonary artery, preventing blood clots. Treatment includes:

- receiving vasodilators, active on relaxing the smooth muscle layer of blood vessels. These include prazosin, hydralazine, and nifedipine;

- medicines-anticoagulants of indirect action, which aims to reduce blood viscosity – acetylsalicylic acid, has dipyridamole, etc.;

- oxygen inhalation during hypoxia and severe shortness of breath;

- the diuretics.

 In very serious cases, transplantation of the heart and lungs.
